We are seeking an experienced Physician Assistant (PA-C) to join our growing Mobile Primary Care team serving seniors in home-based care, assisted living (ALF), skilled nursing facilities (SNF), and senior living communities . This is not a traditional clinic role. This is community-based, field-based medicine focused on geriatrics, chronic disease management, dementia care, and value-based care delivery. Productivity-Based Compensation with Sign-On Bridge What You’ll Do • Deliver Mobile Primary Care in home and senior living settings • Provide comprehensive geriatric assessments and chronic disease management • Manage hypertension, diabetes, CAD, CVA, dementia, and complex Medicare populations • Coordinate care with interdisciplinary teams across hospice, palliative, and behavioral health • Drive patient engagement, retention, and outcomes within a value-based care framework • Practice independently while maintaining high-quality documentation and follow-through What We’re Looking For • Active, unrestricted Physician Assistant (PA-C) license • 1–2 years of clinical experience in primary care or geriatrics • Experience in home-based care, SNF, ALF, or senior living communities preferred • Strong clinical judgment and comfort practicing independently • Interest in leadership, panel growth, and clinical program development • Valid driver’s license (field-based/mobile role) Compensation & Benefits • Guaranteed Base Salary productivity-based compensation model • Sign-on bridge available for full-time providers • Health, dental, vision, and 401(k) • Clear leadership and advancement pathways within a growing organization If you are a Physician Assistant looking to move beyond traditional clinic walls and practice meaningful, relationship-based Mobile Primary Care for seniors , we would love to connect.
