Job Title: Case Manager Profession: RN (Registered Nurse) Specialty: Care Coordination/Case Management Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years License: Required Certifications: BLS Certification Must-Have: Prior experience in Care Coordination Description: Unit information is sent to manager for interview. Candidate must provide two supervisory references at submission. One reference must be within the last 12 months and the second within the last 3 years. Candidates who were formerly employed through an agency will be considered. This position requires prior experience in care coordination. The unit deals with patient case management. Candidates can expect to see 15-20 patients or consultations per day. Familiarity with specific criteria for utilization review is essential. The position involves working with all payers, including uninsured and homeless individuals. Responsibilities include completing initial assessments, discharge planning, and utilizing electronic health record systems. The role includes both utilization review and discharge planning tasks. Collaboration, teamwork, and effective communication skills are necessary for success. Candidates must possess the ability to re-prioritize tasks throughout the day.
