Job Description PRIDE Health is seeking a travel nurse RN Home Health for a travel nursing job in Clearlake, California. Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 07/14/2025 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel ID: 54966904 Shift: 8HR Days 07:30-16:30 Description: Day Shift, 40 hours per week, mileage (government rate) is in addition as travel is needed to see patients. THIS IS FOR HOME HEALTH NOT HOSPITAL. Home Health experience is A MUST/REQUIRED. MUST have HCHB software experience is required. Job Summary: Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items.
