Mercy San Juan Medical Center has received recognition for being a Comprehensive Stroke Center and Center of Excellence for Bariatric Surgery along with Certificates of Excellence in Perinatal Care Hip- and Knee- Replacement.**Responsibilities**Under general medical and supervisory direction, administers the full range of respiratory care modalities in the NICU environment. Initiates and terminates mechanical ventilation per NICU protocol or physician orders. Operates and maintains blood gas analyzer. Provides quality respiratory care services including routine and complex diagnostic and therapeutic procedures designed to evaluate, stabilize, prevent or remedy neonatal pulmonary abnormalities. Utilizes a multi-disciplinary plan of care, including respiratory care protocols, assesses indications, initiates appropriate therapeutic interventions and evaluates anticipated outcomes. Acts as a clinical resource person to department and hospitalpatient care team. Provides education to patients and their families.1.1 Initiates and monitors mechanical ventilation to assure the patient is being ventilated adequately and according to protocol or the physician's written or verbal orders.1.2 Ability to perform arterial punctures for ABG's and other lab work following appropriate infection control technique.1.3 Intubates and extubates neonatal patients as per NICU protocol or the physician's written or verbal orders.1.4 Organizes and gives a comprehensive patient report to the on coming shift.1.5 Approaches the physician to clarify prescribed therapy and recommends modification when indicated.1.6 Assesses and monitors the use of oxygen by performing pulse oximetry to verify medical need for oxygen. Recommends changes in therapy.1.7 Assesses breath sounds, heart rate, respiratory rate, sputum production and clinical condition of the patient before, during and after therapy administration.1.8 Checks physician's orders before administering therapy. Obtains appropriate orders if indicated.1.9 Evaluates efficacy of therapy being given and approaches the physician to recommend appropriate changes.1.10 Consults and discusses with the Nursing Manager of the NICU regarding concerns about the management of patients receiving therapy.1.11 Interprets arterial blood gases correctly and makes/recommends appropriate and timely changes in ventilator settings or therapy.1.12 Performs daily and weekly preventive maintenance on blood gas analyzer. Informs incoming shift if unable to do so.1.13 Performs quality control on blood gas analyzer before it is used. Doesn't operate it if QC values are out of established limits.1.14 Maintains proper and timely documentation of preventive maintenance/quality control procedures.1.15 Fills out timecard accurately and on a daily basis.1.16 Documents accurately on the patient's chart and department's charge master all related information about therapy given.1.17 Entries in the medical record are to be completed accurately and in real time. Informs incoming shift or R.T. head if unable to do so.1.18 Assists with equipment cleaning and stocking of supplies.1.19 Needle thoracentesis under emergent circumstances or as ordered by the physician.1.20 Demonstrates proper and timely documentation of preventive maintenance/quality control procedures1.21 Attends infant births in the Family Birth Center as part of the Neonatal Resuscitation Team to provide support and resuscitative measures as necessary in accordance with NRP guidelines.1.22 Completes department competencies and assigned compliance education in accordance with hospital policy.1.23 Responds to neonatal or pediatric respiratory emergencies of infants less than three (3) months of age in the Emergency Department and Pediatrics as requested as a member of the Neonatal Resuscitation Team.**Qualifications**REQUIRES:+ 2 years minimum acute care experience, with 1 year of critical care experience within a level III NICU.+ Completion of an AMA approved respiratory therapy program+ Licensed Respiratory Care Practitioner with the State of California.+ NRP Certification+ AHA BLS certification+ Neonatal Intubation, STABLE, and NPS.
