  • A hospital near Sacramento is looking for an experienced Care Coordinator for a travel contract.
  • The right candidate will have 2 years of experience.
    • Job Requirements
    • MUST have experience with discharge planning and utilization management.
    • Interqual experience is highly preferred.
    • Minimum of 2 years hospital experience.
      • Schedule Information
      • Shifts depend on the department, but hours are between 8-5pm.
      • Weekend Requirement every other depending on staffing. Would be at least one full weekend per month.
        • Unit Specific Information
        • Unit: Case Management/Care Coordination
        • Floor specific orientation: 5 days of care coordination new employee orientation, unit orientation timeframe depends case by case.
        • # patients seen/consults per day: 15-20
        • Common duties/ Required skills or knowledge: Dealing with all payers, uninsured, homeless. Completing Initial assessments, IQ, DC planning, cerner
        • Do they cover all areas of the hospital? Any outpatient? Any home care? All areas of the hospital, no OP and no Home Care.
        • Scrub Color/Uniform: business causal attire
          • Additional Details
          • For Holiday RTO: Travelers will work 2 of the 3 major holidays (Thanksgiving, Christmas, and New Years).
          • *****MUST HAVE PRIOR DIGNITY EXPERIENCE OR THEY WILL NOT CONSIDER***
          • Holiday time off will be considered, however, not guaranteed.
