As a RN - Case Manager, you will be responsible for coordinating patient care, developing and implementing care plans, and collaborating with healthcare teams to ensure optimal patient outcomes.Responsibilities:

• Coordinate patient care and develop individualized care plans
• Collaborate with healthcare teams to ensure seamless patient care
• Provide education and support to patients and families
• Monitor patient progress and adjust care plans as needed
• Document patient information and care plans in a timely and accurate mannerRequirements:
  • Current California RN license
  • AHA BLS certification
  • 2 years of recent experience in Case Management
  • Strong communication and interpersonal skills
